• Acceptance Letter to Student (Mailed to student applicant within 2 weeks of placement exam and interviews)
• Dress Code

2. Commitment Letter to Parents

(Mailed to parents at same time as Acceptance/Rejection Letter to student)

• Student Information Form (includes Health Form & Photo Release Form)
• Request for Records Form---for previous 2 years' academic records (**ABOVE TWO CORRESPONDENCES MAILED AT SAME TIME**)

No student shall be refused admission to Mariamante Academy on the basis of race, color, sex, national or ethnic origin. Students of religious denominations other than Catholic may be admitted according to standard admission guidelines.
